The rough-in is the distance from the finished wall behind the toilet to the center of the closet flange bolts, measured straight out from the wall. In most homes built in the last several decades this is 12 inches. Some older houses, especially pre-1950s bathrooms, use a 10-inch rough-in, and a smaller number use 14 inches. The rough-in determines how close the tank sits to the wall. A toilet designed for a 12-inch rough-in installed on a 12-inch flange will usually leave about 1 to 2 inches of air gap behind the tank, which is exactly what you want. Model plumbing codes in the United States generally require a minimum of 15 inches from the centerline of the toilet to any side wall, partition, vanity, or tub. That means at least 30 inches of total width for the toilet zone. Accessibility guidelines and comfort-focused designs often push this to 18 inches per side. Check your local code, because some jurisdictions amend these numbers.
Most codes require at least 21 inches of clear floor space in front of the bowl, and 24 inches is the common recommendation for comfortable use. If a door swings into that space, measure with the door open and closed.

The number one cause of a bad gap is buying a 12-inch rough-in toilet for a 10-inch flange, or the reverse. A 12-inch toilet on a 10-inch rough-in physically cannot sit on the bolts without the tank hitting the wall. A thick baseboard or tile ledge can steal an inch or more. Rough-in is always measured from the finished wall surface, so hold your tape above the baseboard. Measuring to the trim is the classic reason a toilet that should fit ends up touching the wall.
Adding cement board and tile, a wainscot panel, or a new layer of drywall behind the toilet moves the finished wall closer to the flange. A rough-in that was 12 inches when the house was built can shrink to 11 inches or less after a remodel, which is enough to make some tanks touch.
Sometimes the plumber simply set the closet bend an inch or two off. This shows up most often in basement bathrooms and DIY additions. The toilet fits, but the gap behind the tank looks wrong or clearances on the sides fail code.

Some toilets are engineered with shallower tanks that tolerate a shortened rough-in. Concealed trapway and one-piece models often need less depth than older two-piece designs. Check the manufacturer's minimum rough-in spec before buying, and do not trust the box photo.
An offset closet flange can shift the bowl position by roughly an inch to an inch and a half. It is a real repair, not a hack, but it slightly restricts the waste path, so pair it with a strong-flushing toilet. Relocating the flange means opening the floor, cutting the closet bend, and re-plumbing the drain to the correct distance. In a basement slab this involves breaking concrete. This is the definitive fix when the flange was set in the wrong place, but it is the most invasive option.
Call a professional if the flange needs to move, if the drain is cast iron or embedded in a slab, if side or front clearances fail code and the room layout has to change, or if a remodel has buried the original flange under new flooring. A plumber can also confirm whether an offset flange is acceptable under your local code, since a few jurisdictions restrict them. Anything that involves cutting the drain line or pouring concrete is worth a licensed set of hands, both for the inspection and for the warranty.
About 1 to 2 inches is normal for a correctly matched toilet and rough-in. Zero clearance means the rough-in is shorter than the toilet was designed for, and more than 3 inches usually means a 10-inch toilet is sitting on a 12-inch rough-in.
It will still flush, but it is not ideal. Contact can transmit vibration, rub the finish, and make the tank lid hard to remove. It also signals that the rough-in and toilet do not match, which is worth correcting at the next replacement.
Always from the finished wall surface, never the baseboard or trim. Hold your tape above the baseboard, or measure to the wall and subtract nothing. Measuring to the trim is the most common reason people buy the wrong toilet.
